INFORMED CONSENT & RELEASE
By signing this document, I acknowledge that I have voluntarily chosen to participate in a program of progressive physical exercise. I acknowledge that physical exercise will put a workload on all bodily systems to improve their function. These systems reactions are unpredictable and some health risks may occur during or after the exercise. I understand that before beginning an exercise program it is recommended that I consult a physician. I agree to hold harmless 24/Fit LLC together with its members, managers, employees, subcontractors, and other agents and free from any and all claims, suits, losses, or related causes of action for damages, including but not limited to such claims that might result from my injury, or death, accidental or otherwise during or arising from the physical activities performed with me or by me. In signing this consent form, I affirm that I have read in its entirety and understand and agree to the above. I agree that all questions regarding this consent form have been answered to my satisfaction.
